Transitions Memory Care
Transitions is a flexible program that is designed to meet the ever-evolving personal care needs of those displaying the initial signs of memory loss. As one’s needs become more advanced, so does the level of care and attention we provide.

Your Monthly Rent Includes the Following:

• Three nutritious homestyle meals each day
• Weekly housekeeping
• Twenty-four hour beverages and snacks
• Individual temperature controls
• Routine room maintenance
• Scheduling and coordination of transportation needs
• Full calendar of activities
• Excellent, well-trained staff, on-site 24 hours each day
• Staffing ratios that exceed licensing requirements

Additional personal care services are available as needed for a fee.

Our program is designed to meet the more intensive care needs of those in the advanced stages of Alzheimer’s. Monthly rent includes a broad range of services that ensure personalized care, comfort, and attention. In addition to the services offered in our Transitions program, we can also provide assistance with mobility, bathing, dressing, and incontinence care.

Additional Services:
Additional services are tailored to meet your personal care and service needs. An initial assessment is completed prior to move-in, evaluated after 30 days, quarterly, and then again as frequently as needed. This assessment will determine how much personal care is required. Charges for added care and services are based upon the care required and the amount of time it takes for staff to provide these additional services.
